Lick the end, place on your areola with the inner disc screwed down so it touches your nipple press against the skin and slowly twist the screw ... it will create a vaccuum and REALLY draw out your nipple. Very intense. Best on hairless skin (you might have to shave a little disc around your nipples but try first)
